Heart Failure Program
Mount Sinai South Nassau’s comprehensive cardiac wellness program delivers trusted, advanced heart care for patients with heart failure, in your neighborhood, backed by the research and physician talent of one of the nation’s leaders in heart care – Mount Sinai Heart.
Heart failure can often be caused by, or intensified by, other conditions that you may not be aware put you at risk. Conditions like diabetes, chronic kidney disease, cerebrovascular disease, high blood pressure, peripheral arterial disease and atrial fibrillation can put you at much greater risk for heart failure.
Mount Sinai South Nassau is certified by the Joint Commission for disease-specific care in Heart Failure. Our Cardiac Wellness program seeks to educate and empower clinicians and patients to reduce their need to reenter the hospital. Patients are educated to practice self-management to improve their condition, and we offer advanced heart failure therapies to allow patients to address heart failure in its early stages, when treatment is most effective.
Through the Cardiac Wellness program, heart failure patients learn to manage their transition back to home following hospitalization or rehabilitation, including improving their understanding of their heart failure and learning self-monitoring and the importance of proper medication use, for more proactive management of their overall health and wellness.
Because heart failure can also be affected by eating patterns and other habits, patients are partnered with experts such as diabetes educators to provide each with an individualized dietary plan they can manage.
Sophisticated monitoring systems are also used to ensure that patients can communicate health information in ways that best fit their needs and lifestyle. High-risk patients may be fitted with implantable telemonitoring devices to allow our physicians to check the status of their health remotely and be alerted to warning signs such as increased pressure in the lungs. A check-in can be performed to ensure that the patient is doing well enough to get through a weekend event or vacation.
Our program is managed by a fellowship-trained heart failure specialist with extensive training and experience in the management of complex heart disease, including heart transplantation and ventricular assist devices.
Our partnership with The Mount Sinai Hospital has the advantage of a continuity of care through the Mount Sinai Health System, with world-class experts in every area of cardiology. If heart transplant or an implanted ventricular assist device is necessary to treat your condition, you can rely on the comprehensive heart transplant program at Mount Sinai Heart to perform the procedure, and then receive integrated ongoing follow-up care at Mount Sinai South Nassau.
