Get With The Guidelines®— Heart Failure and Type 2 Diabetes
To help our patients with heart failure become as healthy as possible, we’ve taken steps to focus on each one. It’s part of the Get With The Guidelines — Heart Failure program. This program focuses on quality care by providing the most up-to-date, research-based guidelines for care of people with heart failure. The goal is to speed up recovery and reduce the need for future hospital stays.
And at Mount Sinai South Nassau, we’ve earned this program’s Gold Plus Quality Achievement Award. The honor is given because of our success in taking specific quality improvement steps, as outlined by the American Heart Association/American Stroke Association. These quality measures include aggressive therapies to help patients reduce their risk for heart failure — like use of proven medicines such as ACE inhibitors/ARBs, beta-blockers, diuretics, anticoagulants, etc.
As part of our goal to help patients with heart failure even after their hospital stay, we also take some important steps before they go home:
- We provide education on managing heart failure and overall health
- Schedule a follow-up visit for them
- Evaluate the need for other steps to help them transition home
