What You Can Expect From Our Experts
Together, members of Mount Sinai South Nassau's geriatric and palliative care team create a plan to provide you with comfort, information and expert medical care at any stage during an illness. We work alongside your medical team while you are in the hospital. We can also help you transition to care after a hospital stay, since we work with a variety of community providers.
Part of our service includes encouraging communication between you, your loved ones and the members of your medical team. So we can address your needs, your choices for care, your wishes and your goals.
Working with your medical team, our services for you and your family include:
- Expert treatment and relief from symptoms such as pain, shortness of breath, tiredness and fatigue, constipation, nausea, loss of appetite, numbness and problems with sleep
- Emotional support
- Spiritual support
- Close communication about your condition
- Information about your choices for medical care
- Coordination of care among all providers while you are in the hospital, as well as after you leave
- Advance care planning, to share your wishes for the types of care you want, and what care you do not want
- Caregiver Support Group
- Referral to hospice experts, for end-of-life care and support
