The DAISY Team Award recognizes that while an idea to achieve better patient and family outcomes may start with one indi¬vidual, it often takes an entire team to implement successfully. The DAISY Team Award is designed to honor collaboration and teamwork by two or more people, led by a nurse, who identify and meet patient and/or patient family needs by going above and beyond the traditional role of Nursing. Annually during Nurses Week, these stand-out teams will be presented with this award.
Criteria:
The team contributed to successful outcomes (either patient or programmatic) for a group or individuals. At least one of these criteria must be met:
- Contribution of time, energy, and talent that resulted in an enhanced outcome for patients, families, or community
- Development and implementation of a product or program, service or activity that supports the Professional Practice Model
- Team collaboration and decision-making processes that contribute to excellent outcomes: quality, professional development, and customer service.
Patients, visitors, nurses, physicians and other employees may nominate a deserving nurse-led team by filling out this form.
Deadline: February 21, 2024, incomplete nominations or nominations received after the deadline will not be considered.
The selection process is based on a review of all nominations and scored using a scoring rubric weighing the criteria by the Nursing Honors council.
