A list of any medications and supplements you take
A list of any allergies
Medical records, including X-rays, if you have them
Contact information for your referring physician and specialists you see, if any
Insurance forms or cards
Any referral or authorization information from your insurance company (which are required by many insurance companies)
Copayment
