Hospitalist Program
Hospitalists: Physicians Who Specialize in Hospital Care
If you’re a patient at Mount Sinai South Nassau, one of our almost 20 “hospitalists” might lead your health care team. Hospitalists are physicians who work on-site at the hospital and specialize in coordinating care during a hospital stay. They are dedicated to providing exceptional care and have advanced training in specific health conditions that commonly lead to hospital stays.
Our hospitalists will establish a plan of care for you and are on call 24/7. They also take the lead in communicating with your loved ones and your providers, including your primary care physician and specialists while you are in the hospital, as well as caregivers as you transition home or to a rehabilitation facility, for example. When you leave the hospital, your hospitalist will provide you with any prescriptions you will need, help arrange any follow-up care and forward a copy of your hospital records to your primary care physician.
Contact us at (516) 632-3666. Or during your hospital stay, dial 3666.
