To help make your admission into our Maternity Unit run as smoothly as possible, here are some suggestions.

Reminder: Please contact your insurance company and enroll your newborn within 30 days of delivery.

What to bring with you

  • Any reports your physician gave you to bring to the hospital
  • Relevant medical insurance information, including insurance cards, pre-certification, and any other documentation required by your insurer
  • Your photo ID, such as a driver's license or any other form of picture ID and proof of address such as utility bill
  • A list of telephone numbers where members of your immediate family can be reached if necessary

What to leave home
It's best that you not bring any valuables with you, such as jewelry, money, etc. Any valuables, however, can be held by the Hospital and claimed by the patient or the next of kin from the cashier's office. No valuable will be released without proper identification.

If you are not sure if we participate with your insurer, please call your insurer for confirmation - and for any special considerations. For example, most patients are required to obtain their Lab and Radiology services at a dedicated facility. Your insurer may require you to pay for any service provided that is not covered by your contract.

If you do not have insurance, or are unable to pay for the hospital services provided, our Financial Assistance Service may be able to help.

Understand your rights as a patient
As a patient in a New York State hospital, you have certain rights and protections guaranteed by state and federal laws. These rights exist to help ensure the quality and safety of your hospital care.

To help you understand these rights, the New York State Department of Health and its Consumer Health Information Council published a booklet "Your Rights as a Hospital Patient." It will be provided to you upon your admission to the hospital. However, if you would like a copy of this booklet before your admission, please call the Admitting Office at 516-632-3990 or contact us.

Pre-register online and save time
Complete our online registration form, then e-mail it to us, or print it out and bring it with you when you register.

Need to file an Acknowledgement of Paternity?
Before signing, please review the "Notice Regarding Your Legal Rights and the Consequences of Signing an Acknowledgement of Paternity" provided by the New York State Department of Health.
