Our team of specialists concentrates on you as a whole person, to address physical, emotional, social and spiritual needs. We work as consultants for you, your loved ones and in collaboration with the medical team caring for you, to improve your quality of life. Palliative Care means “health care treatment, including interdisciplinary end-of-life care, and consultation with patients and family members, to prevent or relieve pain and suffering and to enhance the patient's quality of life, including hospice care” under article forty of [the Public Health Law]

Geriatrics & palliative care can reduce suffering for both the patient and family. At Mount Sinai South Nassau, our focus is helping you to make the best decisions based on your goals and values, by giving you the most up-to-date information about your condition and your choices for care. We work with you to honor your personal wishes for treatment and comfort.

Our geriatric and palliative care experts offer this specialized care to patients and their families seeking treatment for their chronic illness to optimize the quality of one’s life. Palliative care is not hospice care. Hospice care is specific to care and comfort at the end of one’s life. Our team will work to help smooth the transition to that model of care when and if it is appropriate.
